Photo Release Form

By submitting this form, I grant Defined Lines LLC and its representatives, employees, agents, and assigns the irrevocable right to photograph, record, and capture my likeness, image, voice, and/or appearance during the event listed above.

I understand and agree that these photos and/or recordings may be used by Defined Lines LLC for lawful purposes, including but not limited to:

• Marketing and promotional materials

• Website and social media content

• Digital and print publications

• Advertising and portfolio use

• Educational or informational content

I acknowledge that:

• The images may be edited, altered, or combined with other content.

• I waive any right to inspect or approve the final images or materials.

• I waive any right to royalties or compensation related to the use of these images.

• All photos and recordings are the sole property of Defined Lines LLC.

I release and hold harmless Defined Lines LLC and its representatives, agents, successors, and assigns from any and all claims, demands, liabilities, and causes of action arising from the use of the images or recordings.

I understand that this release applies to photography and/or video captured at the above-named event and that participation is voluntary.

If the subject is under 18, this form must be completed by a parent or legal guardian.
